Job summary

Akron Children's Hospital is seeking a Community Health Worker to perform home and community outreach, build trust, and connect families with healthcare and social services. The CHW will coordinate with clinical teams and educate families within a hybrid Barberton setting.

Responsibilities include facilitating goal-based service plans, navigating resources, and documenting outcomes. Requires BLS and CHW cert within specified timelines; HS diploma or GED is required.

Responsibilities

• Conduct home and community-based outreach to build trust with individuals and families.
• Engage individuals using culturally responsive, strengths-based approaches.
• Gathers information to support person-centered service planning.
• Identify social drivers of health, barriers to care, and escalate urgent concerns.
• Partner with individuals and families to set goals and simple action plans.
• Promote self-advocacy and informed decision-making.
• Collaborate with interdisciplinary teams to coordinate services and supports.
• Provide referral assistance and navigate healthcare, social services, and community resources.
• Assist with appointment scheduling, applications, and linkage to needed services.
• Deliver culturally responsive health education, coaching, and emotional support.
• Conduct follow-up to monitor progress and emerging needs.
• Document all activities and outcomes per standards.
• Perform other duties as assigned
