Akron Children's is hiring a Community Health Worker (CHW) for a full-time, hybrid role located at the Boys and Girls Club - Broadway. The CHW will serve as a frontline public health professional building trust with individuals and families to support health, wellness, and access to services.

Responsibilities include outreach, culturally responsive support, care coordination, referrals, education, and thorough documentation. Requires High School diploma, BLS in 90 days, CHW cert within 2 years.

Qualifications

  • High school diploma or GED required.
  • BLS certification required within 90 days of hire.
  • CHW certification through Ohio Board of Nursing required within 2 years.

Responsibilities

  • Conduct home and community outreach to build trust with individuals and families.
  • Engage individuals using culturally responsive approaches.
  • Gather information to support person-centered service planning.
  • Identify social drivers of health and barriers to care.
  • Partner with individuals to set goals and simple action plans.
  • Promote self-advocacy and informed decision-making.
  • Collaborate with interdisciplinary teams to coordinate services.
  • Provide referral assistance and navigate health and social services.
  • Assist with appointment scheduling and linkage to services.
  • Deliver culturally responsive health education, coaching, and emotional support.
  • Follow up to monitor progress and address needs.
  • Document activities and outcomes in compliance with standards.
  • Other duties as assigned

Full-Time: 40 Hours Per Week
1st Shift: 8am - 4:30pm
Monday - Friday
Location: Hybrid - Boys and Girls Club - Broadway
Summary

The Community Health Worker (CHW) serves as a trusted frontline public health professional who has a close understanding of the community served. The CHW builds strong, relationship-based connections with individuals and families to support health, wellness, and access to services. The CHW serves as a bridge between individuals, families, healthcare providers, social service agencies, and community-based organizations to improve health outcomes, reduce disparities, and promote culturally responsive, patient- and family-centered care.

Responsibilities
  • 1. Conducts home and community-based outreach to build trust with individuals and families.
  • 2. Engages individuals using culturally responsive, strengths-based approaches.
  • 3. Gathers information to support individualized, person-centered service planning.
  • 4. Identifies social drivers of health, barriers to care, and escalates urgent concerns appropriately.
  • 5. Partners with individuals and families to establish goals and simple action plans.
  • 6. Promotes self-advocacy and informed decision-making aligned with individual preferences and values.
  • 7. Collaborates with interdisciplinary teams to coordinate services and supports.
  • 8. Provides referral assistance and navigates healthcare, social services, and community resources.
  • 9. Assists with appointment scheduling, applications, and linkage to needed services.
  • 10. Delivers culturally responsive health education, coaching, and emotional support.
  • 11. Conducts follow-up to monitor progress, confirm service connection, and address emerging needs.
  • 12. Documents all activities and outcomes in compliance with organizational, regulatory, and confidentiality standards.

  • 1. Ability to work effectively with individuals and families from diverse cultural, social, and economic backgrounds.

  • 8. Proficiency in MS Office [Outlook, Excel, Word] or similar software is required.
Education and Experience
  • 1. Education: High school diploma or GED required.
  • 2. Licensure: n/a
  • 3. Certification: Current certification in Basic Life Support (BLS) from the American Heart Association is required within 90 days of hire. Community Health Worker certification through the Ohio Board of Nursing required within 2 years of hire.
  • 4. Years of relevant experience: None required. Lived experience with relevant communities/populations is preferred.
  • 5. Years of experience supervising: none
