Job summary

St. Jude Neighborhood Health Center in California is seeking a Community Health Worker to provide Enhanced Care Management to individuals experiencing homelessness and other needs.

The role involves assessing needs over the phone and connecting patients to resources while promoting health education. Qualifications

• Requires High School Diploma or GED.
• 1–2 years of relevant work experience.
• CPR/AED/BLS certification required.
• Must read/write English and Spanish fluently.

Responsibilities

• Assess patients’ needs by phone and connect them to resources.
• Inform patients of test results and provider recommendations; arrange follow-up as requested.
• Assess HIV exposure risk and provide education on testing and prevention.
• Document all interactions in patients' medical records promptly.
• Collaborate with LVN, social worker, physicians, and partner organizations.
