Job summary

Your Health Organization in Blue Ridge, GA and surrounding areas is seeking a dedicated Community Health Worker (CHW) to serve patients in homes and assisted living facilities. The CHW will deliver medications and DME, coordinate visits, and facilitate telehealth to ensure timely access to care and quality service.

The role emphasizes collaboration with care teams, use of RPM tech, and patient education. Travel across Blue Ridge and surrounding areas is required.

Qualifications

• Certified Medical Assistant, Paramedic, or Licensed Nurse preferred and in good standing.
• Phlebotomy experience preferred; willing to train if needed.
• Experience in public health or home health preferred.
• Experience with DME delivery or logistics operations preferred.
• Proficient with technology tools for remote patient monitoring and telemedicine.
• Strong organizational and time management skills; able to multitask.

Responsibilities

• Work with care team to ensure coordinated patient care per plan.
• Communicate effectively by phone, email, and in person.
• Coordinate visits to facilities and patients' homes to support care.
• Deliver medications to patients' homes; perform medication reconciliation.
• Coordinate telehealth visits with pharmacists, specialists, and providers.
• Train patients on RPM equipment and provide ongoing support.
