Job summary

The Community Health Worker (CHW) at UVM Health - Home Health & Hospice works with the interdisciplinary care team to provide a wide range of services and resources to patients and their families, helping coordinate care and connect them to vital community resources.

You will support patients in navigating health services, conduct eligibility and enrollment tasks, document care activities, and engage with families across home, clinic, and community settings to promote healthier outcomes.

Qualifications

• Education and Licensure: High School Diploma
• Successful completion of a community health worker formal training program
• English language proficiency required

Responsibilities

• Support patients in self-assessment of barriers, needs, and goals related to their health status.
• Manage a home visiting caseload and coordinate visits with care teams.
• Documentation and HIPAA compliance in required software.
• Advocate for patients and connect them to resources.
• Assist with transportation and attendance at appointments.
• Collaborate with internal and external partners to optimize care.
• Develop and support care plans aligned with patient goals and transitions.
