Job summary

Qualifications

• Bachelor’s degree preferred (or equivalent relevant professional experience)
• 3+ years healthcare or healthcare customer service experience
• Fluency in English required
• TB clearance required prior to or within 7 days of hire
• Working knowledge of medical/insurance terminology preferred
• Care coordination / discharge planning / care delivery support experience preferred
• Strong documentation, organization, and communication skills

Responsibilities

• Outreach to members, assess needs, and engage eligible participants
• Coordinate communication with members, providers, caregivers, and other stakeholders
• Provide telephonic/email (and when applicable, face-to-face) support aligned to care plans
• Document activities, follow-ups, and coordination steps in care management systems
• Assist with scheduling appointments and connecting members to health care and community resources
• Participate in case conferences and collaborate with interdisciplinary teams
• Identify barriers to care and connect members with appropriate referrals/services
• Escalate cases to higher clinical support (RN/ECM team) as appropriate
• Support Enhanced Care Management (ECM) coordination with external ECM providers (as applicable)

Community Health Navigator - A-Line Staffing is now hiring a Community Health Navigator for a temporary, full-time opportunity supporting care coordination and member support programs.

Pay & Schedule: $42.00 per hour (W2); Schedule: Monday-Friday, 8:00 AM-5:00 PM; Duration: 11/30/2026 through 04/30/2027; Interview: Virtual (1-2 rounds)

Position Overview

Under general supervision from the Non-Clinical Supervisor (Case Management), the Community Health Navigator supports members enrolled in case management and disease management programs. This role focuses on care coordination and helping members access medically covered and non-covered resources (medical, social, behavioral, and community services).

Key Responsibilities

• Outreach to members, assess needs, and engage eligible participants
• Coordinate communication with members, providers, caregivers, and other stakeholders
• Provide telephonic/email (and when applicable, face-to-face) support aligned to care plans
• Document activities, follow-ups, and coordination steps in care management systems
• Assist with scheduling appointments and connecting members to health care and community resources
