Job summary

Cleveland Clinic Health System is seeking a Community Health Worker to connect diverse populations with health and social services in the community setting. You will collaborate with primary care teams to improve access and outcomes, providing education, outreach, and advocacy.

Ideal candidates are skilled at building trust, coordinating resources, and supporting patients in managing chronic conditions. Typical hours are Monday–Friday with some events, in a on-site community center environment.

Qualifications

  • Minimum: High school diploma or GED and valid driver’s license.
  • CHW certification and board nursing CHW certification required.
  • Preferred: Medical Assistant training or certification and EMR knowledge.
  • Strong interpersonal and time-management abilities are essential.

Responsibilities

  • Connect diverse and underserved populations to health and social services.
  • Serve as liaison between patients, families, schools and community agencies.
  • Educate, outreach, counsel and advocate to improve access and outcomes.
  • Assist with scheduling, applications, and navigation of resources.
  • Build trusting relationships and support patients in managing chronic conditions.
  • Document activities and coordinate with care teams.
