The CHW will conduct assessments, coordinate care, provide disease self-management education, and engage in field-based outreach and home visits across several counties. Strong communication and collaboration with care teams are essential.

Qualifications

• High school diploma or equivalent required.
• Graduate of an accredited Medical Assistant program.
• CMA certification preferred.
• Current CPR certification.
• Indiana Community Health Worker certification must be obtained within program-required period.
• At least 1 year of Medical Assistant experience preferred.

Responsabilités

• Provide personalized support to patients and families to navigate healthcare and community resources.
• Conduct comprehensive assessments and social needs screenings to identify barriers and needed resources.
• Coordinate care coordination, referrals, follow-ups, and connection to clinical and supportive services.
• Deliver chronic disease self-management education and support for conditions like diabetes, hypertension, cardiovascular disease, COPD.
• Use patient-centered communication to support engagement and understanding of care plans.
• Assist in coordinating field-based outreach and home visits within the service area.
• Collaborate with clinicians and care teams to address patient barriers and ensure timely responses.
• Maintain accurate documentation of patient contacts, referrals, and outcomes in the designated system.
