Indiana University Health seeks a Community Health Worker to support outreach, education, and navigation services for patients and families at Arnett Hospital in Indiana. You will assess needs using SDOH screenings and coordinate referrals to healthcare services and community resources.

The role emphasizes patient-centered, trauma-informed approaches, collaboration with physicians, nurses, social workers, and community partners to improve health outcomes and equity.

Formación

• Requires a High School Diploma or GED.
• Must obtain Community Health Worker certification within 1 year of hire.
• 0–3 years of relevant experience.
• Ongoing continuing education may be required to maintain certification.
• Requires basic computer skills.
• Requires knowledge of medical, community, and social resources.

Responsabilidades

• Provide outreach, engagement, education, advocacy, and navigation services to patients and families.
• Assess barriers to care and conduct Social Determinants of Health (SDOH) screenings to identify needs.
• Connect patients with healthcare services, community resources, and social service programs, and facilitate referrals.
• Support care coordination activities, including appointment scheduling and follow-up outreach.
• Assist patients in accessing prenatal, postpartum, primary, specialty, behavioral health, preventive, and chronic disease care programs.
• Identify eligible patients and facilitate enrollment in Remote Patient Monitoring (RPM) programs and educate on equipment use.
