Job summary

Mass General Brigham Community Physicians, Inc. seeks a health educator/outreach specialist to deliver advanced health education, outreach, and care coordination to individuals and families in the community.

This role involves working with healthcare providers to ensure patients access appropriate care and support services to improve outcomes. The position requires a Bachelor's degree in Public Health, Social Work or related field, 2+ years in community health outreach, and preferred CHW

Qualifications

Bachelor's Degree in Public Health, Social Work or related field required.
2+ years of experience in community health outreach, health education, SDOH or related field required.
Experience navigating patient social risks and barriers required.
Certified CHW with MA DPH preferred.
Bilingual Spanish skills preferred.

Responsibilities

Conduct in-depth assessments of patient needs, goals, and barriers to achieving good health outcomes.
Provide advanced health education and coaching to individuals and families to promote healthy behaviors and self-management of chronic conditions.
Collaborate with healthcare providers to develop and implement care plans for patients with complex health needs.
Assist patients in navigating the healthcare system and accessing appropriate services.
