Mass General Brigham is seeking a Community Health Worker to deliver advanced health education, outreach, and care coordination to individuals and families in the Salem, MA area. The role collaborates with clinicians to ensure patients access necessary services and achieve better health outcomes.

The position supports a hybrid work model with on-site work at Salem Hospital and occasional travel. Strong communication and bilingual Spanish skills are preferred for serving diverse communities.

Competenze

• Bachelor's degree in Public Health, Social Work or related field required.
• 2+ years of community health outreach, health education or related field required.
• Experience navigating patient social risks and barriers.
• Certified CHW with MA DPH preferred.
• Bilingual Spanish preferred.

Mansioni

• Conduct in-depth assessments of patient needs, goals, and barriers to achieving good health outcomes.
• Provide advanced health education and coaching to individuals and families to promote healthy behaviors and self-management of chronic conditions.
• Collaborate with healthcare providers to develop and implement care plans for patients with complex health needs.
• Assist patients in navigating the healthcare system and accessing appropriate services, including health insurance and social services.
• Provide ongoing follow-up and support to patients to ensure continuity of care and successful achievement of health goals.
