• Preferred 1 year of Healthcare experience with a Bachelor's Degree.

Responsibilities

• Engages in population appropriate communication.
• Gives clear instructions to patients/family regarding treatment.
• Involves family/guardian in assessment and continuing care.
• Identifies physical limitations and deploys intervention when necessary.
• Recognizes and responds to behavioral health issues.
• Interprets population data and plans care accordingly.
• Facilitates referrals to medical homes and community providers.
• Supports follow-up and coordination with care teams.
