Job summary

AmeriCorps is looking for members in South Carolina to serve as Community Health Workers (CHWs) within the SCFCA program. This role involves enhancing health outcomes through effective health coaching and managing a caseload of clients to improve access to health services.

The ideal candidate must be Kershaw County residents, proficient in computer skills, and able to work in diverse environments. Program benefits include a living allowance and an education award upon successful completion of service.

Qualifications

• Must reside in Kershaw County prior to the start of the program year.
• Computer literate and proficient.
• Able to work in a fast-paced, diverse environment.
• U.S. citizen, U.S. national, or lawful permanent resident. DACA recipients are ineligible.

Responsibilities

• Assign and manage a caseload of clients.
• Improve health outcomes through effective health coaching.
• Conduct outreach efforts to increase clinic access.
• Reduce barriers to care for clients.
