Job summary

Monroe Health Center in Forest Hill, WV seeks a Full-time Community Health Worker to help patients access medical, behavioral health, and preventive care services. The CHW connects patients with community resources, supports navigation of the healthcare system, and promotes health education.

The role focuses on identifying barriers such as transportation, housing, food insecurity, and insurance, and conducting outreach, follow-up, and referrals while maintaining patient confidentiality per HIPAA

Qualifications

• High school diploma or GED required.
• CHW certification preferred; willingness to obtain certification may be considered.
• Experience in community outreach, healthcare, social services, or patient navigation preferred.
• Strong communication, interpersonal, and organizational skills.
• Ability to work effectively with individuals from diverse backgrounds.
• Knowledge of community resources and social determinants of health is a plus.
• Basic computer skills and the ability to maintain accurate documentation.

Responsibilities

• Assist patients with accessing medical, behavioral health, and preventive care services.
• Identify barriers to care, including transportation, housing, food insecurity, insurance, and other social needs.
• Connect patients with appropriate community resources and support services.
• Provide health education and encourage preventive care and healthy lifestyle choices.
• Conduct patient outreach, follow-up calls, and referrals as needed.
• Support care coordination and help patients navigate the healthcare system.
• Document patient interactions, referrals, and services provided in the electronic health record or other designated systems.
• Participate in community outreach events, health fairs, and wellness programs.
• Maintain patient confidentiality and comply with HIPAA requirements.
• Perform other duties as assigned.
