Job summary

Northwell Health seeks a Community Health Worker liaison to support vulnerable community members by assessing needs, coordinating referrals, and conducting in-home visits to address material and health challenges.

You will maintain a caseload, serve as a main contact for members, mentor peers, and collaborate with the care team to promote health outcomes and access to community resources.

Qualifications

• High School Diploma or equivalent required.
• 1-3 years of relevant experience, required.

Responsibilities

• Serves as a mentor for Community Health Workers
• Provides oversight in care of members to improve health outcomes and promote well-being
• Acts as a resource for CHW and members
• Provides support and basic health and education about material and general health care and community resources.
• Conducts one-on-one health education, community outreach, regular in-home visits and service coordination.
• Provides various supports, services, entitlements and benefits for members.
• Supports members in developing health management care plans.
• Empowers members by promoting effective approaches, problem solving and lifestyle changes.
• Works to establish positive and supportive relationships and effective work with a diverse group of members.
• Delivers coaching and mentorship on chronic health conditions and self-care.
• Enters client data into software on a daily basis.
• Advocates for members and their families and collaborates with the care team as needed.
