Job summary

HealthLinc is seeking a Community Health Navigator to assist patients with insurance enrollment, navigate coverage options, and educate on the sliding fee scale. You will support care coordination and connect patients with internal and external services to reduce barriers to access.

The role requires CHW/SHIP/CCP certifications or timelines, strong organizational skills, and the ability to work collaboratively with a care team. Remote outreach may be required.

Qualifications

• Education/ training as listed in the job description.
• Certified navigators and CHW certifications within defined timelines.
• Experience with community resources and outreach activities is preferred.
• Experience with insurance navigation and enrollment is preferred.

Responsibilities

• Assist patients with insurance enrollment, reenrollment, and navigation, including Medicaid and Marketplace plans.
• Educate patients on the sliding fee scale and assist with applications and documentation.
• Provide non clinical care coordination, including appointment support and referrals.
• Help patients navigate healthcare, behavioral health, social services, and community resources.
• Address barriers to access such as transportation, cost, language, and literacy.
• Conduct outreach and promote clinic services at community events and health fairs.
• Build relationships with community organizations and stakeholders.
• Maintain required certifications and participate in ongoing training.
