Job summary

Kintegra Health in Thomasville, NC is seeking a Community Resource Advocate to help individuals navigate healthcare programs, connect to community services, and overcome barriers to wellness. The CRA will build relationships, provide education, and assist with enrollment in Medicaid, ACA Marketplace, SHIIP, and other safety-net programs.

Responsibilities

• Connect patients and community members to healthcare and support services.
• Assist with applications and enrollment for Medicaid, ACA Marketplace, Sliding Fee Scale, Medication Assistance Programs, and SHIIP.
• Conduct social health assessments using motivational interviewing techniques.
• Provide health education, outreach, and community presentations.
• Build and maintain relationships with staff, community organizations and stakeholders.
• Participate in health fairs, outreach events, and advocacy initiatives.
• Document services and referrals accurately in required systems.
• Support quality improvement and ensure program compliance.
• Collaborate with staff to maintain efficient workflow.
• Utilize data systems to process referrals and expedite connections to services.
