You’ll explain options in plain language, help with referrals and appointments, and connect patients to essential resources with a patient-centered approach.

Qualifications

• Experience supporting individuals, families, or communities through social or community services.
• Ability to build trusting relationships with people from diverse backgrounds.
• Strong listening, communication, organization, and problem-solving skills.
• Compassionate, patient-centered approach and comfort working directly with people.
• Ability to learn health care and social service systems and documentation practices.
• Ability to maintain confidentiality and follow organizational policies.

Responsibilities

• Meet with patients and families in person to build trusting relationships.
• Explain options in plain language and help navigate health care and social services.
• Assist with referrals, appointments, patient portals, and next steps.
• Connect patients with community resources and explain how to contact or apply.
• Use plain-language and teach-back to aid understanding and decision-making.
• Follow up with patients to ensure access to resources and next steps.
• Document interactions, resources discussed, and follow-up needs.
• Maintain confidentiality and professional boundaries.
