Job summary

SIU Medicine is seeking a program support role that delivers direct services to individuals and works to improve health outcomes for socially vulnerable populations. The position collaborates with medical providers, primary care teams and community partners to connect patients with housing, education, and health resources.

The role emphasizes intake assessments, goal setting, and wrap-around services to ensure continuity of care.

Qualifications

• Coursework in social work or behavioral sciences, or closely related field as measured by conversion table.
• Progressively more responsible work experience in social service/counseling at paraprofessional level under qualified supervision.
• Bachelor's Degree (120 semester hours) equals three years (36 months) in the conversion table.

Responsibilities

• Explain program requirements and guidelines to the general public.
• Develop community resource materials reflecting the local community.
• Conduct comprehensive interviews and intake assessments to determine client needs.
• Assist clients in setting and achieving personal goals in person, by phone or in group settings.
• Assist clients in utilizing resources, managing symptoms of their illness, and developing daily living skills.
• Connect clients to housing, medical care, education, vocational training and other recovery resources.
