Job summary

Terros Health is seeking a Health and Housing Specialist to join our interdisciplinary clinical team at the Priest Drive Recovery Health Center in Tempe, AZ. This full-time role combines community-based housing support with clinical services for adults with substance use, SMI, and dual diagnoses.

The position offers 40 hours/week, with up to 80% fieldwork and 20% office time, mileage reimbursement, a full benefits package including 401(k), and generous PTO.

Qualifications

• High school diploma or GED with 1.5 years of behavioral health experience, or related degree.
• BH-related degrees with relevant experience are considered.
• Experience with NextGen EMR preferred.

Responsibilities

• Provides direct face-to-face community-based services to help clients obtain and maintain housing.
• Conducts screenings, intakes, and assessments to determine treatment goals.
• Coordinates care with family, behavioral and medical providers and community supports.
• Assists clients with connection to physical health services and medical appointments.
• Provides transportation to clients as needed using Terros Health vehicles or personal vehicle.
• Maintains ongoing collaboration and communication with involved parties.
