Family Health Centers of San Diego is seeking a Patient Engagement Specialist II to educate patients and connect medically underserved communities with community health services. You will develop referrals, recruit patients, and collaborate across sites to improve access and outcomes.

The role requires at least 2 years in sales, patient engagement, or outreach; a high school diploma is required, with a bachelor’s degree preferred.

Qualifications

• 2+ years of experience in sales, patient engagement, community education, community health, or outreach programs.
• High School diploma or GED required.
• Bachelor's degree strongly preferred; 4 years of relevant work may substitute for the degree.
• Must have a car and a valid California driver's license; driving is required between sites.

Responsabilités

• Develops, identifies, and provides referrals within and outside of FHCSD and monitors referrals.
• Communicates with and recruits new patients into the clinic’s system of care.
• Leverages community resources to connect new and existing patients with FHCSD.
• Is an effective, productive, and collegial team member.
• Supports the planning, development, execution, and evaluation of patient engagement plans.
