Family Health Centers of San Diego is seeking a Patient Engagement Specialist II to provide patient engagement and health education while connecting medically underserved individuals to community health services. The role emphasizes recruiting new patients, referrals, and effective collaboration with diverse community members.

Qualifications include 2+ years in outreach or related fields, a high school diploma or GED, and a driver’s license for site travel.

Qualificações

• High School diploma or GED required; or equivalent experience.
• Bachelor's degree strongly preferred; 4 years of relevant experience may substitute.
• 2+ years in sales, patient engagement, community education, or outreach required.
• Travel between sites with valid California driver’s license and car is required.

Responsabilidades

• Develops, identifies, and provides referrals within and outside FHCSD and monitors them.
• Effectively communicates with and recruits new patients into the clinic’s system of care.
• Leverages community resources to raise awareness of FHCSD and connect patients to services.
• Participates in planning, development, execution, and evaluation of patient engagement plans.
• Maintains accurate written work and uses computer resources to meet the organization’s demands.
