Job summary

Pacific Health Group is seeking a Street Outreach Advisor to connect communities with Enhanced Care Management, Behavioral Health, and related supports. This field-based role involves meeting partners, expanding access to services, and coordinating referrals in North San Diego County.

You will work in a hybrid setting across Escondido, Oceanside, Vista, and nearby areas, traveling locally to engage with community organizations and individuals while documenting outreach activities in the CRM

Qualifications

• Experience in community outreach, public health, social services, or healthcare navigation.
• Comfort working independently in a fast-paced, community-facing environment.
• Ability to manage outreach activity, relationships, and follow-through across multiple partners.
• Strong organizational skills and comfort using mobile tools and CRM systems.
• Valid driver’s license and ability to travel locally.
• Willingness to work occasional evenings or weekends as needed.

Responsibilities

• Identify and prioritize community-based organizations within assigned local markets.
• Build and maintain referral-generating partnerships across PHG services.
• Spend the majority of the workweek in the field meeting potential partners and the community.
• Identify individuals who may benefit from PHG services and facilitate warm referrals.
• Document outreach activity, referrals, and partnerships in CRM tools.
• Coordinate with internal care teams to track referrals and ensure follow‑through.
