Zusammenfassung

ChristianaCare seeks a Health Coach to support patients in chronic disease prevention and self-management programs. You will apply health behavior change principles, set personalized goals, and collaborate with an interdisciplinary team to reinforce care plans and connect clients with community resources.

The role involves delivering individual and group coaching, documenting progress, and contributing to program improvement.

Qualifikationen

• Bachelor’s degree in health promotion, public health, health education, psychology, social work, nutrition, exercise science, nursing, community health, or related field.
• NBC-HWC and/or CHES preferred.
• Familiarity with Epic, REDCap or data-driven outcome tracking preferred.

Aufgaben

• Assess participant needs using health coaching approaches to support goal setting and action planning.
• Provide health and wellness coaching individually and in groups.
• Use motivational interviewing and active listening to support behavior change.
• Collaborate with healthcare teams and community partners to align care plans.
• Document coaching encounters and outcomes in designated systems.
• Travel to patients' homes or program sites as needed within the service area.
