Job summary

Hamilton Community Health Network in Genesee County, Flint, MI is seeking a Community Health Worker to promote access to health care and improve outcomes for patients and families. You will collaborate with clinical teams to address barriers to care, and help enroll patients in needed services.

The role emphasizes patient advocacy, community referrals, and coordinating with care managers to support continuous patient care. High School diploma required; some healthcare experience preferred.

Qualifications

• High School Diploma
• Two (2) or more years’ experience working in a clinical environment.
• Successful completion of a CHW formal training program within six (6) months of hire.
• Bachelor’s Degree in Healthcare, Community Outreach or Social Services (preferred).
• Experience with Electronic Medical Records (EMR) or client services databases (e.g., Social Solutions, Azara).

Responsibilities

• Establish trusting relationships with patients and families while collaborating with the Hamilton clinical team.
• Schedule patients for Hamilton provider appointments and conduct appointment reminders.
• Provide ongoing interviewing, goal setting and follow-up for patients and families.
• Communicate Hamilton’s mission effectively to medical staff and patients.
• Act as a patient advocate and liaison between patients and community services.
• Assist patients in overcoming barriers to obtaining medical care and services.
• Connect patients with transportation resources when appropriate.
• Assist in enrollment and registration forms to expedite access to care.
• Collaborate with care managers, health educators, recovery coaches, and outreach team.
• Record and track patient outreach and care information in designated systems.
• Maintain knowledge of community resources to assist Hamilton patients and families.
• Provide referrals/linkages for community services as appropriate.
• Assist patients in developing health management plans and self-care strategies.
• Attend staff meetings, trainings and other required sessions.
• Other duties as assigned.
