Job summary

Henry Ford Health seeks a Community Health Worker to bridge healthcare, social services, and Detroit communities. You will promote access to care, help navigate services, and deliver culturally relevant, patient-centered support.

Responsibilities include home visits, telephonic check-ins, referrals to resources, and collaboration with clinical teams to improve health and equity outcomes, with ongoing training and community engagement.

Qualifications

• High school diploma or GED is required.
• Previous CHW experience is preferred.
• Experience in health education or work with underserved populations is preferred.
• Experience conducting home or clinic visits with patients is preferred.

Responsibilities

• Identify and address patients' non-medical and SDOH needs with referrals to social service agencies.
• Collaborate with clinical care teams to ensure continuity of care and support.
• 0Drive patient engagement and retention through outreach and follow-up to reduce no-shows.
• Build trust and foster community connections to bridge healthcare and community resources.
• Provide culturally competent health education and coaching for disease prevention and management.
• Advocate for patients by removing barriers to care and connecting to resources.
• Promote health literacy and help patients navigate systems and self-management.
• Document encounters accurately in the electronic health record and contribute to program evaluation.
• Connect patients with HFH classes, support groups, and related services.
