Job summary

The MetroHealth System in Cleveland, OH seeks a Community Health Worker to support program populations by promoting health, reducing disparities, and improving access to care. Outreaches to individuals, families, and community members to engage in services and connects them to care and resources.

Qualifications

• Required: High school diploma or GED or equivalent.
• One year of experience working with clients in a community-based setting.
• Obtains Community Health Worker certification within one year of hire.
• Life experiences reflective of the community/target population served.
• Reliable transportation to travel to designated community locations.
• Ability to work with diverse client populations and staff at all levels.
• Excellent verbal and written communication skills.
• Outstanding interpersonal, administrative, and organizational skills.
• Proficient with personal computers and Microsoft Office applications.
• Ability to interact effectively with diverse cultural, ethnic, racial, and socioeconomic backgrounds.
• Program-specific: Ryan White/HIV, School Health, Obstetric, Trauma Recovery experience preferred; CHW certifications welcome.
