Job summary

Qualifications

• Must possess a high school diploma or GED and at least one year of community involvement experience.
• Experiences with poverty, housing instability, or other social determinants are valued.
• CHW training certification within 6 months of employment and ongoing compliance.

Responsibilities

• Provide tailored health education and social support to patients and families.
• Identify barriers to care and connect individuals to community resources.
• Engage and enroll individuals in CHW programs and interventions.
• Perform assessments to understand social needs and track outcomes.
• Coordinate with health and social services to improve care access.
• Document activities and referrals in the relevant systems.
• Support performance improvement efforts and cultural competency initiatives.
