Job summary

Mercyhealth Wisconsin and Illinois is seeking a Community Health Worker to provide culturally appropriate health education, outreach, and support in homes, clinics, shelters, and community settings. You will assess patient needs, connect them with resources, and collaborate with the care team to develop and review care plans, helping patients navigate barriers and access care.

Responsibilities

• Document all patient interactions.
• Advocate for patients and help navigate the healthcare system.
• Accompany patients to appointments and assist with forms.
• Provide emotional and social support for living with chronic conditions.
