Job summary

Albuquerque Health Care for the Homeless (AHCH) is seeking a dedicated Community Support Worker to provide trauma-informed, client-centered care to unhoused individuals across Albuquerque.

In this field-based role, you will manage a caseload of 20-30 clients, meet them where they are, and utilize company vehicles to remove barriers to care and housing, following a Housing First philosophy and collaborating with AHCH programs for integrated wrap-around services.

Qualifications

• Must provide trauma-informed, client-centered care to unhoused individuals.
• Experience with harm-reduction approaches in community settings is preferred.
• Strong critical thinking and problem-solving in fast-paced environments.

Responsibilities

• Assess client needs and create individualized service plans.
• Work with AHCH programs to optimize client care and ensure wrap-around services.
• Refer clients to appropriate programs and coordinate with community agencies.
• Conduct client chart documentation in EHR within required timeframes.
• Attend program meetings and clinical trainings as needed.
• Deliver direct service with a substantial portion of visits conducted in homes or community settings.
