• Coordinate care for patients and families, including medical and social needs.
• Complete face-to-face visits and evaluate progress of the care plan.
• Monitor ongoing needs to remove barriers and improve access to care.
• Schedule appointments for members and families; document care plan details.
• Provide updates to families about patient condition and progress; refer to resources as needed.
• Consult with care team to address complex issues.
• Support outreach to identify populations at risk and close gaps.
