Cincinnati Children’s is seeking a Community Health Worker for the Burnet Campus in Cincinnati, OH. The role is a day shift, 40 hours per week, Monday–Friday, 8:00 a.m. to 4:30 p.m., with no evenings, weekends, or holidays.

You will collaborate with doctors, nurses, and social workers to coordinate care, schedule visits, complete documentation, and connect families with resources. A high school diploma and 2–5 years of related experience are required; a valid driver’s license is essential.

Responsibilities

  • Work collaboratively with care team of doctors, nurses, and social workers to coordinate care for patients and families.
  • Complete face-to-face visits, evaluate care plan progress, and communicate results to the care manager.
  • Monitor ongoing needs of patients and families to improve access to care and resources.
  • Schedule appointments for members and families and complete required documentation linked to the care plan.
  • Provide updates to families regarding patient condition and progress; refer families to appropriate resources.

COMMUNITY HEALTH WORKER

Location: Burnet Campus, 3440 Burnet Ave., Cincinnati OH 45229

Shift/Schedule: Day Shift/40 hours per week, Monday-Friday 8a-4:30p. No evenings, weekends or holidays; no on‑call.

Required Qualifications
  • Candidates must have a high school diploma or equivalent
  • 2-5 years of work experience in a related job discipline
  • Have a reliable vehicle/valid driver's license

  • Current Community Health Worker (CHW) certification in the state of Ohio is preferred but not required

  • Provide updates to families regarding patient condition and progress; refer families to appropriate resources for information to answer questions and address needs.
  • Consult with care team members to address complex issues.
  • Supports population outreach to identify population(s) at risk and assist with closing population gaps.
