Qualifikationen

• 1-3 years of related work experience required.
• Excellent verbal and written communication skills.
• Experience with computer systems including web-based applications and some Microsoft Office applications.

• Determine resources to meet patients' and families' needs and develop a service plan.
• Act as case manager to coordinate care with medical providers.
• Maintain a caseload with high daily contacts and document in the EMR.
• Advocate for patients in communications and facilitate access to entitlements.
• Develop relationships with community/state agencies to negotiate complex systems.
• Make home visits as needed and expand expertise in areas like trauma or HIV.
