Job summary

Orchid Health in Sandy, Oregon, is hiring a Community Health Worker to help patients navigate services, engage in care, and support care management within an integrated team. The role emphasizes building trusting relationships and advancing health equity in the local community.

Qualifications

• Minimum of high school diploma or GED; higher education preferred
• One year of relevant experience with underserved populations in social services or community settings
• Basic computer skills; familiarity with EHR systems preferred
• Ability to initiate and maintain positive relationships with patients, staff and organizations
• Strong communication skills and ability to listen and engage respectfully

Responsibilities

• Help patients navigate community services and resources, creating a welcoming environment
• Assist OHP patients in engaging in care and maintaining insurance coverage
• Support providers and care management team in delivering whole-person care
• Act as a local liaison and patient advocate within the community
• Establish trusting relationships with patients and families while encouraging care goals
• Conduct outreach to newly assigned patients and those at risk of losing insurance to support continuity of care
