Job summary

Access: Supports for Living is seeking a Community Habilitation Worker to provide person-centered supports in home and community settings in Howard Beach, Queens. This role emphasizes independence and community integration, with a 6-hour weekly schedule at $19 per hour.

Responsibilities include implementing the Habilitation Plan, documenting services, and promoting client rights and personal growth. Must meet education requirements and communicate effectively in English.

Qualifications

• Must have a High School Diploma or GED.
• Ability to communicate effectively, both written and oral.
• Strong interpersonal and motivational skills with people with disabilities.
• English literacy required.

Responsibilities
