Job summary

New York Families for Autistic Children is seeking a part-time Community Habilitation Worker in Howard Beach, Queens, NY. The role provides person-centered supports to individuals with developmental disabilities and emphasizes independence and community integration.

Qualifications

• High School Diploma or GED required.
• Ability to read, write, speak and understand English.
• Ability to work independently and handle multiple tasks.
• Strong interpersonal, listening, communication and motivational skills.
• Proficiency with computer and email.

Responsibilities

• Provide services in home and community per Habilitation Plan.
• Document services contemporaneously per requirements.
• Monitor safety and welfare; notify supervisor as needed.
• Assist people to build relationships and advocate for needs.
• Complete required paperwork daily and monthly summary notes.
• Support community integration and attendance checks on transportation.
• Attend staff meetings and trainings.
