Job summary

Access: Supports for Living in New York is seeking a Community Habilitation Worker to provide person-centered supports in home and community settings, promoting independence and community inclusion.

Qualifications

Responsibilities

• Provide services in home and community per Habilitation Plan.
• Document services contemporaneously per requirements.
• Monitor safety and report concerns to supervisor.
• Assist individuals to build community integration.
• Complete required paperwork and attendance records.
• Attend staff meetings and trainings.
