Job summary

Access: Supports for Living in Queens, NY is seeking a Community Habilitation Worker to support individuals with developmental disabilities in home and community settings. You will provide guidance, training, and encouragement to promote independence and community integration, following individual plans and safeguarding requirements.

Qualifications

• High School Diploma or GED required.

Responsibilities

• Provide services to persons in home and community per Habilitation Plan.
• Document Habilitation services contemporaneously.
• Monitor safety and welfare of people served.
• Assist and support people served to build positive relationships in the community.
• Provide needed training and encouragement for personal health care as required.
• Complete all required paperwork and daily notes.
