Job summary

Cooper Green Mercy seeks a Medical Social Worker to support adults in a clinical and non-clinical setting. You will assess psychosocial needs, coordinate services, advocate for patients, and connect them with community resources to improve outcomes.

You will collaborate with interdisciplinary teams, manage documentation, and participate in ongoing training. The role is full-time, daytime hours, at the CGM Cooper Green location.

Qualifications

• Bachelor’s degree in Social Work is required.
• 2–5 years of experience in social work, social services, case management or related healthcare setting.
• LMSW licensure (Licensed Medical Social Worker) required.
• Master’s degree in Social Work preferred.

Responsibilities

• Engage with communities to identify social determinants of health impacting access to care and outcomes.
• Provide support within Prescription Assistance, Resource Center, and Social Work.
• Educate patients about transportation services and eligibility criteria for resources.
• Collaborate with community organizations and resources to address social issues affecting health outcomes.
• Conduct psychosocial assessments and provide case management interventions tailored to each patient and family.
• Coordinate and facilitate services to meet complex needs of individuals and families.
• Advocate for patients’ rights and help navigate the healthcare system.
