Qualificações

• Bachelor’s degree in Social Work required; LMSW licensure required.
• 2–5 years in social work or related healthcare setting.
• Licensed Medical Social Worker in good standing.

Responsabilidades

• Assess psychosocial needs and plan interventions.
• Advocate for patient rights and navigate healthcare system.
• Coordinate services with social services, prescription assistance, and resource centers.
• Document patient encounters accurately per SOPs.
• Work with multidisciplinary team to support patient outcomes.
• Educate patients about resources and transportation services.
