Job summary

Urban Strategies, Inc in Fayetteville, NC seeks a Family Support Specialist II to provide comprehensive case management and coordinate with community services under the CNI Grant. You will work with social workers, relocation and employment specialists to support families and seniors toward economic self-sufficiency.

The role requires intake, home visits, development of service plans, and advocacy for residents while maintaining confidentiality and data entry in LEARN.

Qualifications

• Bachelor’s degree in social work or related field required.
• Minimum three years’ experience as a case manager or service connector for low-income households.
• Commitment to strengths-based, family-focused service planning.
• Ability to handle privileged information confidentially.
• Team player with strong communication and collaboration skills.
• Willingness to attend evening and weekend events and travel as needed.

Responsibilities

• Conduct intake and assessments for enrollment into the case management program.
• Conduct home visits, office visits, and community site visits to collect information.
• Develop, implement, monitor, and modify Development Plans with a multidisciplinary team.
• Assist residents in navigating social service organizations and public agencies.
• Identify service gaps and advocate for residents across caseloads.
• Maintain privacy and confidentiality of all household information.
• Enter and track resident data in LEARN to document progress in real time.
• Attend community meetings with partners and social service entities.
• Participate in Urban Strategies program meetings and project-related meetings.
• Occasionally work nights and/or weekends; travel as required.
• Perform administrative duties such as budget monitoring, reporting, and billing documentation.
