Job summary

Qualifications

• High school diploma or GED is required.
• CPR, Basic First Aid, and medication administration certification required (may be obtained upon hire).
• Valid driver's license required.
• Related experience preferred.
• Evenings and/or weekends availability.

Responsibilities

• Assess individual needs and goals; plan and carry out a schedule of community activities aligned with the person-centered plan.
• Provide direct support in the community while promoting inclusion, meaningful participation, and personal choice.
• Maintain accurate documentation and records.
• Implement supports that protect health and safety.
• Maintain confidentiality and professional conduct; collaborate with individuals served, families/caregivers, and community partners.
