Job summary

The Judge Group is seeking a Field-Based Outreach Advocate to support health plan members facing Social Determinants of Health barriers. The role operates co-located within the Onslow County DSS office and requires daily travel across Onslow County and nearby areas.

You will connect members with community programs, coordinate with clinical teams, and guide them to appropriate providers and services to improve health outcomes.

Qualifications

• Bachelor’s degree in Public Health, Social Work, Rehabilitation Counseling, or a related healthcare field.
• Minimum of 1 year of direct experience with target populations or community-based organizations.
• Valid driver’s license and ability to travel 4–5 days per week.
• Strong digital proficiency with administrative tools and remote communication software.

Responsibilities

• Engage members with severe SDOH challenges to set actionable goals and immediate next steps.
• Guide members to healthcare providers, social services, and community support programs.
• Collaborate with Healthcare Management to align action plans with case management.
• Serve as liaison between external referral sources, local agencies, and the health plan.
• Manage high-volume multichannel inquiries (phone, text, fax, digital) with detailed member notes.
• Travel daily across Onslow County and surrounding areas for field engagements and site visits.

Work Environment & Schedule

• Schedule: Monday–Friday, 8:00 AM – 5:00 PM (Approved overtime as needed).
• Job Type: Contract to hire (Medical, dental, and vision insurance are available to qualified candidates who meet eligibility requirements.)
• Location Category: Field-Based (In-person 5 days/week).
• Primary Worksite: Co-located at the Onslow County DSS office. Candidates must reside within a reasonable commuting distance.

Position Overview

Serves as the primary contact and advocate for health plan members facing significant Social Determinants of Health (SDOH) barriers. Works co-located within the Onslow County DSS office to conduct field-based community outreach, evaluate individual member needs, and establish personalized action plans. Connects members with vital community programs, health care resources, and clinical teams to streamline care delivery and drive positive health outcomes.

Key Responsibilities

• Engage members identified with severe SDOH challenges, working collaboratively to establish actionable goals and immediate next steps.
• Guide members in selecting appropriate healthcare providers, social services, and community-based support programs aligned with their unique needs.
• Collaborate with Healthcare Management (HCM) to ensure member action plans integrate smoothly with existing case management efforts.
• Serve as the principal communication liaison between external referral sources, local agencies, and the health plan.
• Manage high-volume multichannel inquiries (phone, text, fax, and digital queues) while maintaining detailed member notes simultaneously.
• Travel daily across Onslow County and surrounding areas to conduct field-based member engagements, site visits, and community outreach.

Required Qualifications

• Bachelor’s degree in Public Health, Social Work, Rehabilitation Counseling, or a related healthcare field.
• Minimum of 1 year of direct experience working with specific target populations or community-based organizations.
• Valid driver's license and ability to travel 4–5 days per week to worksites, member homes, and local provider offices.
• Strong digital proficiency with administrative tools, electronic management platforms, and remote communication software.

Preferred Qualifications

• 2+ years of experience in marketing, job development, job placement, or social work.
• Working knowledge of Vocational Rehabilitation (Voc Rehab) systems and state assistance programs.
