Qualifications

• Bachelor’s degree in social work or related field preferred.
• 3+ years of public/community health experience
• 1-2 years of case management or home health care experience
• Evidence of specialized training and experience as a Doula
• Must have valid Florida driver’s license and reliable transportation.

Responsibilities

• Process incoming referrals for program enrollment.
• Assist participants in developing individualized care plans and family support plans.
• Carry out case management activities including home visitation monthly or as needed.
• Educate participants on prenatal care options.
• Provide health education, including waiting two years between pregnancies.
• Screen participants for depression, IPV, and substance abuse and provide appropriate interventions.
• Administer the Ages and Stages Questionnaire with participants and infants.
• Collaborate with community partners to meet participant needs.
• Perform other duties as assigned.
