Hill Country MHDD Centers in Uvalde County is seeking a Community Diversion Coordinator to support individuals with mental health challenges involved with the criminal justice system. The role includes assessment, care planning, case management, and liaison with court staff.

Flexible hours, travel required, crisis intervention training, and adherence to HIPAA and agency policies. A sign-on bonus of 4,000 is offered to new hires and state licensure credentials are encouraged.

Qualifications

• Bachelor's degree in social, behavioral, or human services and QMHP-CS credential eligibility.
• Valid Texas driver’s license and auto insurance.
• Must become certified in PMAB, First Aid and CPR within three months of employment.
• Preferred: 3–5 years of mental health community-based experience.

Responsabilités

• Attend court staffing meetings and hearings.
• Coordinate services and case management for justice-involved individuals.
• Complete assessments and develop person-centered recovery plans.
• Ensure on-time documentation and 95% adherence to CANS/ANSA timelines.
• Travel to service delivery sites and maintain client confidentiality.
