Job summary

Element Care in Lowell, MA is seeking a Registered Dietitian to deliver nutrition education and assessments for PACE participants. You will collaborate with the IDT to plan, implement and evaluate care plans aligned with the PACE model.

The role is full time, Monday–Friday, 8am–4pm, with no weekends or holidays and strong benefits. Required: MS in Nutrition (preferred), RD through CDR with MA licensure, and 2 years of community experience including geriatrics.

Qualifications

• Master of Science Degree in Nutrition (preferred).
• Registered Dietitian through CDR with current Massachusetts licensure.
• Two years of experience in a community setting.
• One year of experience with geriatric population.
• Covid Vaccinated Preferred.

Responsibilities

• Assess participants’ nutritional status at enrollment, annually, and as needed.
• Share participant information with Interdisciplinary Team.
• Plan dietary supplements for high-risk participants.
• Report observations to ADH staff clinicians.
• Participate in family meetings/case conferences as needed.
• Provide follow-up treatment and teaching as needed.
• Make home and community visits as required.
• Interact with participants, caregivers, and families professionally.
• Interact with other providers and community professionals courteously.
• Maintain accurate records and timely documentation.
• Pass a fit test; respirator seal required; no facial hair interfering with seal.
• Frequent local travel.
• Perform other duties as assigned.
