Job summary

Element Care PACE is seeking a Registered Dietitian to provide nutrition education and assessments for PACE participants. You will participate in the IDT to plan and evaluate care plans aligned with the PACE model. The role is full time, Monday–Friday, 8am–4pm, with no weekends or holidays and a strong benefits package.

You’ll assess nutritional status, coordinate with the team, plan supplements, and document changes. Travel locally is required.

Qualifications

• Master of Science Degree in Nutrition (preferred).
• Registered Dietitian through CDR and current Massachusetts licensure.
• Two years of experience in a community setting.
• One year of experience with geriatric population.
• Ability to convey information clearly and succinctly to staff.
• Ability to lead independently and work well with others.

Responsibilities

• Assess participants’ nutritional status at enrollment, annually, and as needed.
• Share participant information with the Interdisciplinary Team.
• Plan dietary supplements for high-risk participants.
• Report observations to ADH staff clinicians.
• Participate in family meetings/case conferences as needed.
• Provide follow-up treatment and teaching.
• Conduct home and community visits as needed.
• Interact professionally with participants, caregivers, and providers.
• Maintain timely records and documentation.
• Frequent local travel; pass fit test for respirator mask.

Job description

The Registered Dietitian provides nutrition education and assessments to the PACE participants. He/she participates as a member of the IDT in the planning, implementation and evaluation of care plans that meet the objectives, standards and policies of the PACE model of care. The Registered Dietitian works in tandem with the Team to achieve the PACE Mission of assisting frail elders to remain living in the community for as long as they can do so safely. This position is full time M-F 8am to 4pm with no weekend or holiday hours and great benefits.

• Assesses the participants’ nutritional status at the time of enrollment, annually thereafter, and as needed.
• Shares relevant participant related information with the Interdisciplinary Team.
• Helps with planning for dietary supplements for high-risk participants.
• Reports observations and participant changes, including but not limited to behavioral changes, to the ADH staff clinicians.
• Participates in family meetings / case conferences, as needed.
• Provides follow-up treatment and teaching, as needed.
• Makes home and community visits, as needed.
• Interacts with participants and caregivers / families in a professional and courteous manner.
• Interacts with other providers and community professionals in a courteous and professional manner.
• Responsible for the maintenance of appropriate records and documentation in a timely manner.
• Ability to pass a fit test. Position requires mask where seal is critical. Incumbent is required to not have facial hair that interferes with a tight seal of the respirator.
• Frequent local travel.
• Performs other duties as assigned

Job Specification:

• Master of Science Degree in Nutrition (preferred)
• Registered Dietitian through CDR and possess current state licensure in the Commonwealth of Massachusetts.
• Two (2) years of experience working in a community setting.
• One (1) year of experience working with the geriatric population.
• Ability to convey information clearly and succinctly with all employees, so that relevant information is communicated appropriately and in a timely manner.
• Ability to lead independently and to be able to work well with others.
• Ability to work within a team concept and to be considerate of co-workers and colleagues.
• Ability to pay attention to detail, to be a self-starter, and to complete tasks on time.
• Ability to be able to quickly recognize situations/problems before they become acute, and to be able to implement effective solutions and understand consequences.
• Covid Vaccinated Preferred.
