Job summary

Harbor Health's Elder Service Plan/PACE Program in Massachusetts is seeking a full-time Dietitian to join our nutrition team. The role is hybrid, with two days on site and community-based care with mileage reimbursement.

You will implement the nutrition program, conduct comprehensive assessments, and guide participants and families on nutrition while collaborating with IDT and food services to ensure regulatory compliance.

Qualifications

• Bachelor’s degree in Food and Nutrition or Dietetics; Master’s degree preferred.
• Massachusetts Registered Dietitian license; BLS certification required.
• Proficiency with Microsoft Office 365 (Word, Excel, Outlook, Teams).
• At least 1 year of nutrition counseling and assessments.
• At least 1 year of geriatric experience.
• Excellent written and oral communication skills.
• Current ServSafe – Allergen or MA-Allergen certification (or within 3 months).
• Current ServSafe Food Handler certification (or within 3 months).

Responsibilities

• Perform initial and annual nutrition assessments and document in EMR.
• Develop individualized nutrition plans and communicate to participant, IDT, Dietary Aides and family.
• Provide nutrition education and guidance to participants and families.
• Coordinate with Dietary Aides and food vendors to ensure plan compliance.
• Arrange for home-delivered meals and specialized meals when needed.
• Ensure kitchen operations meet OSHA, DPH, PACE, CMS regulations.
• Lead safety trainings on food handling and sanitation for ESP staff.
