Qualifications

• Experience delivering interventions based on a treatment plan.
• Ability to collaborate with a multidisciplinary team.
• Familiarity with recovery-oriented, person-centered practice.

Responsibilities

• Participate as a team member and deliver life-skills interventions.
• Identify community resources and promote PS participation in mainstream opportunities.
• Provide training to staff on community inclusion and available entitlements.
• Assist PS with obtaining benefits and transportation, housing and healthcare.
