The Community Coach facilitates community-based activities and supports people in Fello’s Day Services. All activities and supports are individualized and aligned to the Person-Centered Plan. The Community Coach will work with an assigned group of one or more people with a disability to promote independence and personal growth. The Community Coach will provide transportation to and from activities for people served they are assigned to support. The Community Coach must meet the qualifications of a Direct Support Professional and complete all training.
